Uterine cancer, also called endometrial cancer when it begins in the lining of the uterus, is the most common type of cancer affecting the female reproductive organs. When abnormal cell growth occurs in this lining, it can develop into cancer. Most uterine cancers are adenocarcinomas arising from glandular cells. Because abnormal uterine bleeding is an early warning sign, uterine cancer is often detected at an earlier stage compared to many other cancers, which significantly improves the outlook.
Symptoms
Unusual vaginal bleeding is the most common and early symptom of uterine cancer. This may include bleeding between periods, after menopause, or unusually heavy periods. You might also notice unusual vaginal discharge that is not blood. Pelvic pain or pressure, pain during intercourse, and difficulty urinating may occur as the cancer progresses. Unexplained weight loss and fatigue can also develop. Any post-menopausal bleeding should be investigated as soon as possible.
Causes
Elevated estrogen levels not balanced by progesterone are closely associated with uterine cancer risk. Obesity, hormone therapy using estrogen alone, late menopause, and a history of irregular periods can raise risk levels. Women who have never been pregnant or who have polycystic ovary syndrome may face higher risk. Family history and genetic mutations such as Lynch syndrome are also significant factors.
Diagnosis
Diagnosis typically involves a pelvic examination, transvaginal ultrasound, and an endometrial biopsy. A procedure called dilation and curettage may be performed to collect a larger tissue sample. Imaging tests help determine the extent of the disease and plan treatment.
Treatment
Surgery to remove the uterus, known as hysterectomy, is the primary treatment for most cases. Radiation therapy, hormone therapy, chemotherapy, and targeted therapy may also be used.
