Head and neck cancer is a broad category that includes cancers arising in the mouth, throat, larynx, salivary glands, nasal cavity, and paranasal sinuses. The most common type is squamous cell carcinoma, which develops in the cells lining the moist surfaces inside the head and neck region. These cancers can affect vital functions such as speaking, swallowing, breathing, and chewing. Head and neck cancers account for a notable proportion of cancer cases in India and are frequently associated with tobacco and alcohol use. Early identification of unusual lesions or changes in the mouth or throat can make a significant difference in treatment outcomes.
Symptoms
Symptoms vary depending on the location of the cancer. A persistent sore or ulcer in the mouth or throat that does not heal is a common sign. A lump in the neck, mouth, or throat, changes in voice or hoarseness, difficulty swallowing or chewing, and pain when swallowing are also important symptoms. Nasal obstruction, frequent nosebleeds, persistent ear pain, and unexplained weight loss may occur. Loosening of teeth without a dental cause and numbness in the face or mouth are additional signs that warrant prompt evaluation.
Causes
Tobacco use, including smoking and chewing tobacco, and heavy alcohol consumption are the most significant risk factors. Human papillomavirus infection, particularly HPV-16, is increasingly linked to oropharyngeal cancers. Prolonged sun exposure increases risk for cancers of the lip and skin of the face. Poor dental hygiene, exposure to certain wood dust and chemical fumes, and previous radiation to the head and neck are also contributing factors.
Diagnosis
Diagnosis involves a thorough physical examination of the head and neck, nasopharyngoscopy or laryngoscopy to visualize the throat and airways, and biopsy of suspicious lesions. Imaging tests such as CT, MRI, and PET scans assess the tumor size, location, and spread. HPV testing may be performed for certain throat cancers.
Treatment
Treatment may include surgery, radiation therapy, chemotherapy, targeted therapy, and immunotherapy, often in combination. Reconstruction and rehabilitation are often part of the care plan.
