Esophageal cancer develops in the esophagus, the long muscular tube that carries food and liquids from the throat to the stomach. It is an important but often overlooked organ, and cancer here can significantly affect the ability to eat and swallow. There are two main types: squamous cell carcinoma, which arises in the flat cells lining the esophagus and is more common in the upper and middle portions, and adenocarcinoma, which develops in glandular cells and is more frequently found in the lower portion near the stomach. Esophageal cancer tends to be diagnosed at later stages, making awareness of early warning signs critical for better outcomes.
Symptoms
Difficulty swallowing, known as dysphagia, is the most common and distinctive symptom. You may initially have trouble swallowing solid foods and later liquids as well. Unintended weight loss is common due to reduced food intake. A sensation of food getting stuck in the chest or throat, regurgitation, heartburn, and indigestion that persists are also reported. Chest pain or pressure, hoarseness, a chronic cough, or hiccups that do not resolve may develop as the cancer progresses. Any swallowing difficulty should be evaluated without delay.
Causes
Chronic acid reflux leading to a condition called Barrett's esophagus significantly increases the risk of adenocarcinoma. Smoking and heavy alcohol use are strongly linked to squamous cell carcinoma. Obesity, a diet low in fruits and vegetables, and very hot beverage consumption are also contributing factors. Long-standing achalasia and certain caustic injuries to the esophagus increase risk as well.
Diagnosis
Endoscopy with biopsy is the key diagnostic procedure that allows direct visualization and tissue sampling. Imaging tests such as CT scan and PET scan assess the extent of disease and involvement of lymph nodes or other organs. Endoscopic ultrasound helps determine how deeply the tumor has invaded the esophageal wall.
Treatment
Treatment may include surgery, chemotherapy, radiation therapy, targeted therapy, and immunotherapy, often used in combination.
