Endometrial cancer begins in the endometrium, which is the inner lining of the uterus. It is the most common type of uterine cancer and one of the most frequently diagnosed gynecological cancers. The endometrium undergoes regular changes during the menstrual cycle under the influence of hormones, and abnormal hormonal conditions can contribute to cell changes that lead to cancer. Endometrial cancer is more commonly diagnosed in postmenopausal women, but it can occur at any age. When detected early, which is often the case because of the early warning symptom of abnormal bleeding, the outlook is generally favorable with appropriate treatment.
Symptoms
Abnormal uterine bleeding is the most prominent symptom and should always be taken seriously. In postmenopausal women, any vaginal bleeding is considered abnormal. In premenopausal women, unusually heavy periods or bleeding between periods are warning signs. A watery or blood-tinged vaginal discharge may also occur. As the cancer advances, pelvic pain, pain during intercourse, or pain when urinating may develop. Unexplained weight loss and fatigue are other possible symptoms.
Causes
High levels of estrogen not offset by progesterone are a key contributing factor. Obesity is closely linked to endometrial cancer as excess fat tissue produces additional estrogen. Older age, never having been pregnant, early menstruation, late menopause, and diabetes are known risk factors. Lynch syndrome and other hereditary conditions also increase the likelihood of developing this cancer.
Diagnosis
Diagnosis involves a pelvic examination, transvaginal ultrasound to measure the thickness of the endometrium, and an endometrial biopsy. If the biopsy is inconclusive, a hysteroscopy or dilation and curettage procedure may be done. Imaging tests help determine staging.
Treatment
Hysterectomy is the primary treatment and is often curative in early-stage disease. Radiation therapy, hormone therapy, chemotherapy, and targeted therapy may be added based on the stage and risk factors.
