Cervical cancer develops in the cells of the cervix, the lower part of the uterus that connects to the vagina. It is one of the most common cancers in women in India and is largely preventable with screening and vaccination. The vast majority of cervical cancers are caused by persistent infection with certain strains of the human papillomavirus, commonly known as HPV. Most HPV infections clear on their own, but in some cases, they lead to cellular changes that can progress to cancer if untreated. Regular screening through Pap smears and HPV tests plays a vital role in detecting precancerous changes early, which can be treated before cancer develops.
Symptoms
Early cervical cancer may produce no symptoms, which is why regular screening is so important. As the cancer develops, you might notice unusual vaginal bleeding, such as between periods, after intercourse, or after menopause. Vaginal discharge that is watery, pink, or foul-smelling may occur. Pelvic pain, pain during intercourse, and lower back pain may develop as the disease progresses. Advanced cancer can cause problems with the bladder or bowel, including difficulty urinating or blood in the urine.
Causes
Persistent infection with high-risk strains of HPV, particularly HPV-16 and HPV-18, is responsible for the overwhelming majority of cervical cancers. Smoking weakens the immune system's ability to fight HPV and increases cancer risk. Having multiple sexual partners, early sexual activity, and a weakened immune system from conditions like HIV increase susceptibility. Regular screening can detect precancerous changes before they progress.
Diagnosis
Pap smear, also called a Pap test, is a routine screening tool that detects abnormal cervical cells. An HPV test is often done alongside it. If abnormal cells are found, a colposcopy and biopsy are performed for confirmation. Imaging tests are used to determine staging.
Treatment
Treatment depends on the stage and may include surgery, radiation therapy, chemotherapy, or a combination.
