Bladder cancer originates in the cells lining the inside of the bladder, an organ that stores urine before it leaves the body. It is among the more commonly diagnosed cancers and can affect people of all ages, though it is more prevalent in older adults and more common in men than women. The most frequent type is transitional cell carcinoma, also called urothelial carcinoma, which begins in the cells lining the bladder wall. Other types include squamous cell carcinoma and adenocarcinoma. Bladder cancer has a high recurrence rate, which makes follow-up monitoring particularly important even after successful initial treatment.
Symptoms
Blood in the urine, known as hematuria, is the most common early sign of bladder cancer. The urine may appear pink, red, or dark brown. You might not feel pain when this occurs. Frequent urination, pain or burning during urination, and an urge to urinate without being able to are also symptoms. In advanced stages, lower back pain, pelvic pain, and difficulty urinating may develop. Any blood in the urine should be reported to a doctor immediately.
Causes
Smoking is the single most significant risk factor for bladder cancer. Exposure to certain industrial chemicals, including those used in dye, rubber, and leather industries, increases risk. Chronic bladder irritation from repeated infections or the use of catheters can also contribute. Older age and a family history of bladder cancer are additional risk factors. Arsenic exposure through contaminated water has also been associated with higher risk.
Diagnosis
Diagnosis involves urine tests, cystoscopy, where a thin tube is used to examine the inside of the bladder, and biopsy. Imaging tests such as CT urography and ultrasound help assess the extent of disease. Urine cytology tests look for cancer cells in the urine.
Treatment
Treatment depends on the stage and grade of the cancer. It may include transurethral resection, intravesical therapy, surgery, radiation, chemotherapy, targeted therapy, or immunotherapy.
